## Entry standard and manager check

- Work owner and reference: [client/internal / ticket/project/code].
- Category and contract boundary: [agreed definition].
- Work date and duration: [recorded / reconstructed estimate].
- Result: [checked / found / changed / tested].
- Next step: [action / receiving owner].
- Billing treatment: [separate policy decision].

Manager check: can a reviewer trace the entry, classify it the same way and tell whether the task is resolved? Keep logs and secrets in the appropriate restricted system, with safe references in the note.
